All of our intake paperwork can be completed online through our scheduling software, Jane.

If you are unable to find an email with link to filling out your paperwork, please download, print, and complete the following three forms before your first appointment. Alternatively, please plan to arrive 15 minutes prior to your first appointment to complete your paperwork in the clinic.

Once you are signed up on our Jane scheduling software you will have the option to get reminders of appointments via text and/or email. Please note the following key information around SMS/Text communication:

Text message communications. You may choose to enroll in text messaging through Jane to receive appointment reminders. By submitting your phone number and selecting a box to opt-in, you agree to receiving texts from Bluebird wellness. Normal messaging rates apply and the frequency of messages may vary. Mobile carriers are not liable for delayed or undelivered messages.

No mobile information will be shared with third parties/affiliates for marketing/promotional purposes. All other categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.


Opt-out of text message communications. You may opt out of text messaging at any time by replying to any message with STOP or contacting us at bluebirdwell@gmail.com. This will end the communications from that particular phone number. You may continue to receive service-related and other non-marketing text messages from other phone numbers which you have enrolled with Bluebird, and you may opt out of those in a similar fashion.
